Core Services and Approach
Patient-Centered Care: We prioritize the needs and preferences of each patient, ensuring that treatment plans are individualized to support sustainable recovery. We collaborate with patients to set achievable goals, enhance motivation, and address any co-occurring conditions that may impact their health and well-being.
Trauma-Informed Care: Recognizing the role that trauma can play in substance use, our program incorporates principles of safety, trustworthiness, peer support, and cultural competency into every interaction. This approach aims to reduce the likelihood of re-traumatization and to foster a therapeutic environment conducive to healing.
Qualified and Credentialed Staff: Our program is supported by a team of licensed and credentialed professionals, including experienced clinicians, nurses, physician assistants, and medical staff, who are dedicated to providing evidence-based treatment. Our staff members are trained in the latest best practices: medication for opioid use disorder (MOUD) as well as harm reduction and motivational interviewing techniques.
Collaborative Approach: Montefiore Mount Vernon SATP partners with a range of community agencies and correctional facilities to facilitate seamless transitions of care, including managing referrals, intakes, and admissions from the Department of Corrections. This helps to ensure that individuals receive consistent, uninterrupted support.
Comprehensive Support Services: In addition to MOUD, we offer a range of supportive services such as counseling, group therapy, and case management to address the multifaceted needs of our patients.
Physical at Admission and Annual Physicals: We also provide Hepatitis C testing, STI testing, along with routine laboratory services.
