Schaffer Extended Care Center offers short-term rehabilitation/subacute care and skilled long-term care at our 150-bed facility. Schaffer offers a wide range of healthcare and social services for medically frail seniors and those with chronic conditions or disabilities.

Health Services

For more than 30 years, Schaffer’s rehabilitation program has been helping cardiopulmonary, orthopedic and surgery patients return to their homes and families. 

Our short-term program specializes in cardiopulmonary rehabilitation, post-surgery and stroke recovery, and fracture, fall and frail patient care.

Burke at Schaffer Extended Care Center

Through Schaffer’s partnership with Burke Rehabilitation Hospital, our physical, occupational, and speech therapists provide comprehensive rehabilitative care and individualized treatment programs for patients onsite.

We are focused on helping patients rapidly recover and regain their function and independence. Outpatient and inpatient physiotherapy staff members trained by Burke experts work to ensure the therapeutic care meets the high standards Burke is recognized for.

Support Services

Most patients stay at Schaffer for at least a few weeks. So we cultivate an environment that meets the needs and interests of each patient. More than personalized care plans, this includes spaces where friends and family can visit comfortably, and therapeutic recreation that offers engaging activity for all levels of ability.

As part of Montefiore Health System, we believe an excellent way to help reduce your anxiety and provide an environment to promote the best outcome is through information and education. If you are in need of our services, please contact our Social Workers at 914-365-3807 for more information. 

Interpretation Services

We provide, free of charge, language interpreter services, telecommunication and amplification devices, and sign language services to persons who are deaf and/or hearing impaired. Please ask your healthcare professional for assistance. 

Customer Services

At the Schaffer Extended Care Center, the doctors, nurses and staff will treat you and your family as partners in your own care. One important way that you can be involved is to speak up. Ask questions, voice your concerns and don’t be afraid to raise any issues relating not only to your care and treatment, but also to overall services.

Below are steps that will help you stay safe, get the information you need, ask the right questions and interact effectively with your doctors, nurses and hospital staff. Please remember to:

  • Write down any questions you have
  • Choose a family member to communicate with the doctors and staff
  • Keep a list of doctors you see and the medications they prescribe
  • Reach out to the Social Services Department to address questions and concerns

Step Up and Speak Up

The most positive care outcomes occur when patients are proactive. Be sure to:

  • Speak up. Ask questions and voice concerns. It’s your body, and you have a right to know.
  • Pay attention. Make sure you’re getting the right treatments and medications.
  • Educate yourself. Learn about the medical tests you get and your treatment plan.
  • Find an advocate. Pick a trusted family member or friend to be your advocate.
  • Know what meds and why. Understand what medicines you take and why you take them.
  • Check before you go. Use a hospital, clinic, surgery center or other type of healthcare organization that meets The Joint Commission’s quality standards.
  • Participate in your care. You are the center of your healthcare team.

Take the Connecticut Turnpike S to Exit 16, New England Thruway S (I-95).

Take Parkways to Robert F. Kennedy (Triboro), Throgs Neck or Whitestone Bridges.

Follow signs to North Avenue. Turn right on North Avenue and go one block to Lockwood Avenue.

Turn left on Lockwood Avenue and go three blocks to Glover Johnson Place.

Option 2:

Parking

Patient and visitor street parking is available on Guion Place. Visitor parking is also available in the visitors lot located on Lockwood Avenue between VanGuilder Street and Washington Avenue.

Hospital visitor parking is also available on Glover Johnson Place, directly across the street from the main entrance to Montefiore New Rochelle. Rates and additional information can be found here.
